Povzetek

Izhodišča. Povezanost proteinurije z napredovanjem popuščanja delovanja ledvic je predmet številnih raziskav v zadnjih letih. Izkazalo se je, da je obilnejša proteinurija povezana s hitrejšim popuščanjem delovanja ledvic. Zaviralci angiotenzin konvertirajočega encima (ACE) so zdravilo izbire pri bolnikih s kronično ledvično boleznijo in arterijsko hipertenzijo, razen znižanja krvnega tlaka imajo tudi antiproteinurični učinek ter zaščitno delovanje na ledvično tkivo. Glede na izsledke zadnjih raziskav imajo podoben učinek tudi antagonisti receptorjev za angiotenzin II. V zadnjem času je bilo objavljenih tudi nekaj raziskav o vlogi kombiniranega zdravljenja pri zmanjšanju proteinurije.

Bolnik. Avtorja prikazujeta primer bolnika s Henoch-Schönleinovo purpuro in proteinurijo, pri katerem sta s kombiniranim zdravljenjem uspela učinkovito zmanjšati proteinurijo.

Zaključki. Zaviralci ACE in antagonisti receptorjev za angiotenzin II imajo približno enak učinek na zmanjšanje proteinurije. Učinek kombinacije obeh zdravil na proteinurijo je zaenkrat manj raziskan, vendar so rezultati prvih raziskav obetavni.
